| F758 | Unnecessary Psychotropic MedicationsThe CMS survey tag cited when a psychotropic medication lacks an adequate documented indication, monitoring, or justification for its dose and duration. |
| F605 | Chemical RestraintsThe CMS survey tag for a resident's right to be free from a psychotropic used as a chemical restraint: a drug given to control behavior rather than to treat a diagnosed condition. |
| F757 | Drug Regimen Free of Unnecessary DrugsThe CMS survey tag requiring each resident's drug regimen to be free of unnecessary drugs, including any without a documented clinical indication or given in excessive dose or duration. |
| GDR | Gradual Dose ReductionA stepwise, documented attempt to lower a psychotropic dose toward the lowest effective amount. CMS expects regular attempts unless they are clinically contraindicated. |
| PRN | As-needed (medication order)A medication ordered to be given only as needed rather than on a fixed schedule. Psychotropic PRN orders carry stricter time limits and documentation requirements. |
| EMR | Electronic Medical RecordThe digital chart a facility uses to record resident medications, conditions, and care. NeuroLTC reads from the EMR but does not replace it. |
| IDT | Interdisciplinary TeamThe group of nursing, medical, pharmacy, and social-work staff who jointly plan and review a resident's care, including psychotropic use and dose-reduction decisions. |
| AIMS | Abnormal Involuntary Movement ScaleA standardized screening tool used to detect and track involuntary movements such as tardive dyskinesia in residents on antipsychotic medications. |
| BAA | Business Associate AgreementA HIPAA-required contract that governs how a vendor may handle protected health information. NeuroLTC executes one with every facility customer before processing any PHI. |
| FHIR | Fast Healthcare Interoperability ResourcesAn industry-standard format for exchanging healthcare data between systems. NeuroLTC uses it to read from EMRs such as PointClickCare. |
| PHI | Protected Health InformationIndividually identifiable health information protected under HIPAA, such as a resident's name, diagnoses, or medications. |
| DON | Director of NursingThe nurse leader responsible for a facility's clinical operations and, in practice, much of its day-to-day survey readiness. |
| SNF | Skilled Nursing FacilityA facility providing 24-hour skilled nursing and rehabilitation care, subject to CMS survey and certification requirements. |
| LTC | Long-Term CareOngoing services for residents who need help with daily living or medical care over an extended period. It is the setting NeuroLTC is built for. |
| CMS | Centers for Medicare & Medicaid ServicesThe federal agency that sets and enforces nursing home regulations, including the F-tags cited during surveys. |
| KPI | Key Performance IndicatorA tracked metric used to gauge performance, such as the share of psychotropic orders with a current indication or completed GDR attempts. |
| PCC | PointClickCareA widely used long-term care EMR. It is the first system NeuroLTC integrates with, via a read-only FHIR connection. |
